Received media can land in your library automatically
By default, media received in chats may be saved to the camera roll, which places images sent by other people into your photo library and any cloud backup it feeds.
The setting exists in the app's storage and data or chat settings, and turning it off stops future saves.
Copies exist in several places at once
The same image can exist as a message attachment inside the app, an entry in the app's own cache, a file in the camera roll, a copy in a cloud photo library, and an item inside the app's chat backup.
Turning off auto-save addresses the third of those going forward and none of them retrospectively.
